This kidney has a profound change.

Question 1: Describe the lesion.

The renal pelvis is dilated and the medulla is mostly absent. Renal pillars are visible and are prominent.

Question 2: What disturbance of growth is illustrated here?

Atrophy.

Question 3: What has happened here?

This is a case of hydronephrosis. Hydronephrosis is the result of obstruction in the ureter, leading to build up of urine proximal to the obstruction. Glomerular filtration can usually persist in the face of the obstruction, leading to a gradual increase in pressure in the renal pelvis, the collecting ducts, and renal tubules. The increased pressure causes of atrophy of the tubular epithelium, and can also compress blood vessels, leading to ischemic damage and scarring. This is an example of pressure atrophy.

Please note you would not be expected to recognize this lesion as atrophy at this stage of your training. This example is provided for your interest and learning only.
